If insufficient cortisol is present and the person tested responds to an ACTH stimulation test, then the problem is likely due to insufficient ACTH production by the pituitary. If the person does not respond to the ACTH stimulation test, then it is more likely that the problem is based in the adrenal glands. If the adrenal glands are underactive, due to pituitary dysfunction and/or insufficient ACTH production, then the person is said to have secondary adrenal insufficiency . If decreased cortisol production is due to adrenal damage, then the person is said to have primary adrenal insufficiency or Addison disease .

Because steroids are lipophilic, they diffuse easily through the cell membranes, and therefore have a very large distribution volume. In their target tissues, steroids are concentrated by an uptake mechanism which relies on their binding to intracellular proteins (or " receptors ", see below). High concentration of steroids are also found in adipose tissue, although this is not a target for hormone action. In the human male, adipose tissue contains aromatase activity, and seems to be the main source of androgen-derived estrogens found in the circulation. But most of the peripheral metabolism occurs in the liver and to some extent in the kidneys, which are the major sites of hormone inactivation and elimination, or catabolism (see below).

Individuals with Cushing syndrome will experience rapid weight gain in the face, abdomen, and chest. Often doctors will notice this because of the individual's slender arms and legs compared to the heavy weight in the core of the body. Cushing syndrome also causes a flushed face, high blood pressure, and changes in the skin. Osteoporosis and mood swings are also a factor considered with Cushing disease.

As a result of this, I have had 2/3 of my pituitary gland removed, my adrenal gland removed (they had to break and remove a rib to take it out) and each time I got it I gained between 60-70 lbs. Blood pressure and cholesterol were extremely high. Now I have the opposite problem, where my cortisol is extremely low and I need to take steroids to keep it at an acceptable level.
